There are generally two categories of medical insurance known commonly as indemnity or managed-care plans. The major differences between the two categories are evident in how you choose your provider, how out of pocket costs are handled and how bills get paid. For example, under an indemnity policies you usually can choose any doctor or hospital that you wish when seeking services. Under a managed-care plan, you either select a primary care physician who treats and directs all health care for you or you are given a list of preferred providers from which you can chose any provider. Under indemnity plans, you often to pay upfront and are reimbursed after a deductible is applied. Under a managed care plan, providers agree to perform services for patients at pre-negotiated rates and usually the provider handles the claims processing for you.

In 2001, Pennsylvania created the adultBasic program which provides health insurance for adults between the ages of 19 and 64 who meet certain eligibility requirements and who do not otherwise have healthcare coverage. It is administered by the Pennsylvania Insurance Department and funded by proceeds from the state’s tobacco settlement. The program offers basic health benefits including preventative care, physician services, diagnosis and treatment of illness or injury, in-patient hospitalization, out-patient hospital services, emergency accident and medical care. The good news is that there are over fifty thousand individuals currently enrolled in the adultBasic program. The bad news is that there is limited funding available for the program so as of July 2008 there were over one hundred thousand individuals on a wait list eagerly seeking coverage under the adultBasic program.

Pennsylvania's also offers a Children's Health Insurance Program (CHIP) program that provides health insurance to all uninsured children and teens who are not eligible for or enrolled in the Medical Assistance program. To be eligible for CHIP, a child must not be eligible for Medical Assistance or have any other health insurance, must be under age 19, must be a US citizen or permanent legal alien, and must be a Pennsylvania resident. For many families, CHIP is free. Families with income exceeding the free CHIP limits pay low monthly premiums and co-pays for some services. The state also recently enacted new legislation that requires health insurance providers to provide autism health insurance coverage for children under 21 years old.
